Canyon is looking for a Community Care Manager/LPN. Working within
the framework of Vista Prairie Communities’ values of Caring,
Respect, Collaboration, Innovation, and Stewardship, the Community
Care Manager/LPN is responsible for building on and helping find
purpose for each resident, helping them connect to their purpose
and enhance their quality of life. This position involves leading,
developing, and training direct care staff, as well as creating and
managing their schedules. Additionally, this role involves
monitoring the delivery of services provided to residents. Your key
accountabilities will be to deliver and manage care through the
Aspire Lifestyle Model to honor residents and enhance their quality
of life. You will also be accountable for engaging residents using
the Aspire 5 pillars of wellness including Social, Physical,
Intellectual, Emotional, and Spiritual. Some of your resident care
responsibilities will include answering resident emergency calls
promptly, assisting in transporting residents to/from appointments,
activities, meals, and social programs as necessary. Some of your
staff scheduling, onboarding, and mentoring/performance
responsibilities will include assisting with identifying and
documenting any changes in staffing or resident conditions and
confirming all staff complete their daily duties. As an LPN, your
LPN duties will include: - Assists DOHS in orienting, training, and
mentoring new staff. - Assists with the startup of services,
transitions, and discharges of residents. - Performs delegated
tasks and other duties as assigned. - Plans, assigns, directs, and
evaluates the work of unlicensed staff members and ensures they
perform their duties and responsibilities promptly. - Works under
the direction of the DOHS to monitor clients' needs and changes in
condition, conducting focused assessments when needed, and
reporting the results to appropriate parties. - Carries out proper
charting practices and documentation procedures. - Reports changes
or problems in resident conditions or other concerns about clients.
- Reviews client care plans under the direction of the immediate
supervisor. Together we can make a meaningful impact on the lives
of our residents! Schedule: This position is Full-time, M-F Days,
On call rotation. What you will need: - Must be registered and
licensed with the State of Wisconsin as an LPN and must maintain
current, unencumbered license with the Board of Nursing. - Training
in health-related disciplines and/or experience required. - Must be
18 years or older. - Experience managing/leading a large department
with complex scheduling, orienting, mentoring, disciplining, and
enhancing employee engagement. - Excellent communication skills,
